Yoğun Bakımda COVID-19 ve Yoğun Bakımdan Kliniğe Transfer Süreci

Özet

COVID-19 pandemisi, yoğun bakım ünitelerinde hem hasta hem de sağlık çalışanı güvenliğini korumak adına hasta bakım prosedürlerinde köklü değişiklikleri beraberinde getirmiştir. Hastaların önemli bir kısmı hastalığı hafif atlatırken, bir bölümü ciddi solunum yetmezliğiyle yoğun bakım tedavisine ihtiyaç duymaktadır. Yoğun bakım sürecindeki uzun süreli hareketsizlik; kas güçsüzlüğü, nöromiyopati, kontraktürler ve derin ven trombozu gibi ciddi fonksiyonel kayıplara yol açarak "Uzamış COVID-19 Sendromu" ve "Yoğun Bakım Sonrası Sendromu" gibi kronik tabloları tetiklemektedir. Bu süreçte fiziksel ve kognitif yetileri geri kazandırmayı amaçlayan erken rehabilitasyon ve özellikle hava yolu temizliği hayati önem taşımaktadır. Yoğun bakımdan kliniğe transfer süreci ise vital bulguların stabilitesi, akciğer lezyonlarının gerilemesi ve laboratuvar parametrelerinin normale dönmesi gibi kriterlere bağlı, son derece dinamik ve hassas planlanması gereken bir aşamadır. Bulaş riskinin önlenmesi amacıyla transferlerin güncel prosedürlere uygun, kişisel koruyucu ekipman kullanan donanımlı ekiplerle yapılması, hastane içi kontaminasyonun engellenmesi ve klinik ekiple eksiksiz bilgi paylaşımı hem hasta güvenliği hem de sağlık çalışanlarının korunması açısından kritiktir.

The COVID-19 pandemic has brought fundamental changes in patient care procedures to protect both patient and healthcare worker safety in intensive care units. While a significant portion of patients experience the disease mildly, some require intensive care treatment with severe respiratory failure. Prolonged immobilization during intensive care leads to serious functional losses such as muscle weakness, neuromyopathy, contractures, and deep vein thrombosis, triggering chronic conditions like "Prolonged COVID-19 Syndrome" and "Post-Intensive Care Syndrome". During this process, early rehabilitation aiming to restore physical and cognitive abilities and especially airway clearance are of vital importance. The transfer process from intensive care to the ward is a highly dynamic stage that must be planned meticulously, depending on criteria such as the stability of vital signs, regression of lung lesions, and normalization of laboratory parameters. To prevent transmission risk, performing transfers in accordance with current procedures with equipped teams using personal protective equipment, preventing in-hospital contamination, and sharing complete information with the ward team are critical for both patient safety and the protection of healthcare workers.
