COVID-19 Tanısı Olan Erişkin Hastalarda Temel ve İleri Yaşam Desteği Uygulaması

Özet

COVID-19 tanısı veya şüphesi olan erişkin hastalarda ani gelişen solunum ve kalp durması (kardiyak arrest) durumunda uygulanan Temel Yaşam Desteği (TYD) ve İleri Yaşam Desteği (İYD) protokolleri, SARS-CoV-2 virüsünün yüksek bulaşıcılık riski sebebiyle kurtarıcı güvenliğini merkeze alacak şekilde güncellenmiştir. Kardiyopulmoner resüsitasyon (KPR) sırasındaki göğüs kompresyonları ve havayolu müdahaleleri asılı damla partikülleri üreterek bulaş riskini ciddi oranda artırdığından, müdahale ekiplerinin N95/FFP2/FFP3 maske, siperlik, koruyucu önlük ve eldivenden oluşan kişisel koruyucu ekipmanları (KKE) eksiksiz kullanması hayati önem taşır. Profesyonel olmayan sivil kurtarıcıların bulaş riskini önlemek adına ağızdan ağıza yapay solunum yapmaması, yalnızca hastanın ağız ve burnunu kapatarak kesintisiz göğüs basısı uygulaması ve varsa otomatik eksternal defibrilatör (OED) kullanması tavsiye edilmektedir. Profesyonel sağlık ekipleri için ise balon valf maske kullanımında yüksek filtre özellikli viral HEPA filtrelerin entegre edilmesi, entübasyon işlemlerinin deneyimli personelce video laringoskop gibi teknolojilerden yararlanılarak hızlıca yapılması ve odadaki personel sayısının asgari düzeyde tutulması önerilir. Ayrıca, entübe edilmemiş pron (yüzüstü) pozisyondaki hastalarda KPR öncesi hızlıca supin (sırtüstü) pozisyona geçilmesi gerekirken, entübe hastalarda sırt bölgesinden kompresyon uygulanabileceği belirtilmektedir. Bu güncel kılavuz önerileri, pandemi şartlarında hasta sağkalımını optimize ederken sağlık çalışanlarının ve sivil kurtarıcıların güvenliğini en üst düzeyde korumayı hedefler.

Basic Life Support (BLS) and Advanced Life Support (ALS) protocols implemented during sudden respiratory and cardiac arrest in adult patients with diagnosed or suspected COVID-19 have been updated to center rescuer safety due to the high transmissibility risk of the SARS-CoV-2 virus. Since chest compressions and airway interventions during cardiopulmonary resuscitation (CPR) generate aerosolized droplets and significantly increase transmission risks, it is of vital importance for response teams to fully utilize personal protective equipment (PPE) consisting of N95/FFP2/FFP3 masks, face shields, protective gowns, and gloves. To prevent transmission, lay rescuers are advised against performing mouth-to-mouth artificial respiration, and should instead focus on continuous chest compressions by covering the patient’s mouth and nose while utilizing an automated external defibrillator (AED) if available. For professional healthcare teams, integrating high-efficiency viral HEPA filters during bag-valve-mask ventilation, performing rapid intubation by experienced personnel leveraging technologies like video laryngoscopy, and keeping the number of staff in the room to a minimum are highly recommended. Furthermore, while non-intubated patients in the prone position must be quickly flipped to the supine position before CPR, chest compressions can be performed from the back between the scapulae for intubated patients. These current guideline recommendations aim to optimize patient survival under pandemic conditions while maintaining the safety of both healthcare professionals and lay rescuers at the highest level.
